Every individual applying to take the GED test must fill out Attachment A:

Main Application for GED Test - Attachment A (PDF)

If you are under the age of 19 on the day of the GED test you must fill out Attachment B.

Attachment B for GED Test Applicants Aged 17 or 18 Years Old (PDF)

If you lost your GED Diploma or transcript and would like another transcript or a duplicate diploma, follow the instructions in Attachment H.
